The COVID-19 Risk Assessment Survey is intended to gather data to help us understand the risks that COVID-19 poses to communities due to their particular lifestyles. Based on the data gathered, we will identify practical steps to mitigate such risks and help these communities develop long-term strategies to keep themselves and their families safe.
Beneficiaries of such strategies would be people who are at high risk to COVID-19 including those living in multiple occupancy houses or in unstable housing situations, those on low-income and ethnic minorities. They will be reached through outreach work, schools and key contacts in the community.

The survey is intended for anybody living in Rugby (although it can be filled by anybody living in Warwickshire).
Ideally at least one member of a household must fill the survey.
Please fill the survey yourselves.
Please encourage your friends, colleagues, neighbours, family members and colleagues in your offices to fill the survey.
If you are serving a particular clientele or a community group, please ask them to fill it in.
If you are working in a care setting, supported accommodation and other such settings, please ask the occupants to fill the survey.
